details on the 59,$Q.
-a-day
chronic care copayment
he announced last month.
"We believe we have
designed a system of
payment, that will lead to
more equitable and• ef-
ficient use of the hearth
care system," Timbrell
said.
"Since 1972, long-term
patients in nursing homes
have made a daily con-
tribution toward their
maintenance. It was
referred to as a
copayment. Long-term
patients in chronic care
facilities, however, were
not subject to a similar
charge. That discrepancy
tended to discourage
movement of people from
chronic care facilities to
nursing home, even when
that level of care would
have been more ap-
propriate.
"It is vital that the high
quality of Ontario's
health tare system be
maintained. That means
making the most efficient
use of our resources," he
said.
There has been a good
deal of support for this
expansion of the
copayment principle:

of the Ontario Legislature
on Health Care Financing
and . Costs, endorsed by
all parties, recom-
mended: "...that
moderate per diem
charges be imposed upon
chronic care patients in
The Joint Advisory
Committee of the Ontario,
Government and the
Ontario Medical
Association on Methods
to Control"Health Care
-Costs, known as the
Taylor Report, recom-
mended: "...that a daily
fee be levied for chronic
hospital care after three
months' stay...."
The Ontario Advisory
Council on Senior Citizens
recommended: "...that
financial „ incentives be
built into proper
placement of the elderly.
It should not be finan-
cially advantageous to
remain at . the
costliest...Ievel of
care...."
"We were guided, in
developing the policy, by
the following con-
siderations," said
Timbrell.
"The exemptions must
be easily understood and
simple to administer; the
cost of maintaining a
family in the community
must be taken into
consideration for both the
patient with dependents
and the family of the
dependent patient;' and
payment ' of the charge
should not create
financial difficulties for
any individual, or his or
her family. For example,
no one should have -to
apply for welfare."
"While we believe the
exemptions provide relief
from payment for all
those who need it, we
realize, of course that
there may be exceptional
circumstances.
Therefore, we have put in
place a system, under the
legislative authority of
the Family Benefits Act;
through which these rare
cases can be reviewed
and resolved," he said.
CHRONIC CARE
EXEMPTIONS:
DETAIL
All patients under the
age of 18 are fully
exempted.
Patients will be
exempted if they are in
receipt of Family
Benefits or General
Welfare Assistance.
When these patients are
in chronic care, they are
provided a $45 per month
"comfort allowance" and
not charged a
copayment.
All patients in chronic
care beds' will be
exempted from the
copayment for the first 60
days after admission. A
