"In our experience, we have found out that the recipients of this service are men and women over the age of 60 or 65," points out Mrs. Thind. Since Huron County has a ' large elderly population, the clinics , have served the needs of this age group. The purpose of the service is to provide health counselling, health—assessment including screening for hearing and teacl}ing the use of , hearing aids, anaemia test and general health sur- veillance, foot care and introduction to physical fitness. These clinics are run in all, major towns and special clinics twice a year in areas away from towns. "The elderly find talking to nurses about their concerns most useful," says Mrs. Thind. "Group discussions ar `also held with films as well as speakers present." � The physician - public health nurse attachment program carried on with the family doctors in the county, has mainly been set up through the community hospitals. Family physicians refer cases out of their practice as well as those admitted to hospital for the public health nurse to follow up. The idea for this formal • arrangement is to provide continuity of care. "Due to the very fact that public health nurse is knowledgeable about resources and -other agencies in com- munity, she can assist the , person in matters that are contributory to his -her health which would be difficult to do for a physician in his busy practice," says Mrs. Thind. .. "At the same time, a nurse is able to assess the situation in the home as it relates to other fariii-ly members and reinforce the treatment or action the physician has• requested the patient to follow," she adds. While the public health nurse functions as a liaison person in the hospital, her major functions are assessment and follow-up as , well as referral to' other agencies. Public health nurses also provide liaison for the home care program.. "In, total, we• serve' 38 elementary schools including one room Mennonite sch.Qols and five, secondary schools," Mrs: Thind states. "We try to provide public health nursing service one half day to 400 pupils, a recommended average by school health task force. In addition, the dental, immunization, /hearing and vision program staff cover 3 �+ the schools through their program in both elementary and secondary schools." At the present time, Huron County Health Unit has adopted the Computer Assisted School Health Program (CASH) to assist in maintaining proper record system. The records for the school enterants at kindergarten level are created after thorough screening and physical examination. "It is - our attempt to transfer the health records of our school age population which will hold records of children with no obvio.is health problems in mater printouts and recommend" periodical follow-up for those with known health needs," says Mfs. Thind. In elementary school, public health nurse duties involve teacher -nurse communication follow-up of referrals from teachers to home, physicians and other agencies as well as of those children already known to the public health nurse. She is also involved in communicable` disease control and in identifying and counselling .on physical, emotional -and learning problems. In the area of health education, the public health nurse functions mainly as a resource person to teachers as well as doing , some classroom teaching" when requested. In secondary schools, the major function is counselling, being liaison between home, school and other agen- cies and providing health education, particularly in area of birth control, sex education and venereal disease. During the last year and ahalf, youth clinics have been introduced in schools. "This seems to assist students mostly in the area of counselling for basic and intense emotional problems,", says Mrs. . Thind. Clinics also offer public health nurse counselling, hearing screening, vision screening, anaemia screening, throat swabs, blood tests for V.D., R.H. factor and irhmunity against german measles and hepatitis and health counselling'. By age three, an audiometric test is possible. This tests overa wider range of sounds and is used at preschool clinics anti on older children at baby clinics. One child of four years in Huron 'who did not speak was thought to be retarded but was found to be deaf. He is now at Robarts School, London, and progressing quite well. While the tests takes only 1- 2 'minutes, the counselling given to those adults in- terested enough to ask for a hearing 4,test at the Adult Clinic can take some time. Most of them complain of not being able to hear in a crowd or group situation. "As we age, we lose the higher tones which aid in understanding speech and at this time, we must learn to listen, watch.,the speaker and. to use the hearing that we have left," Mrs. Steepe said. "Some are wondering if a hearing aid would help and it must be' explained that a hearing aid is only an am- plifier and as such only makes it harder to hear at meetings," Mrs. Steepe added. Most hearing is lost by age 60-65 so persons who are getting along without too much trouble by then probably do 'not need a hearing aid.
