Do you know someone who has made exceptional contributions to our community, enriching it through his or her powerful
leadership? Someone who deserves to be recognized for everything she or he has done? Now is your chance! To celebrate its
tenth anniversary, the Grand Bend Community Foundation is hosting the Community Leaders of the Decade Awards.
It's a once -in -a -decade opportunity to say "thank you" to the people who help make Grand Bend grand!
Who are we looking for'?
The successful nominees, through their personal efforts and achievements, have significantly improved the quality of life in Grand Bend during the past decade. Their
contributions were innovative, energetic, and persistent— above and beyond the 'call of duty.' These individuals consistently exhibit exemplary conduct and positive attitudes
toward their community and fellow citizens.
'Who can nominate?
Anyone who lives in the Greater Grand Bend area may make one nomination.
Who can he nominated''
Anyone who:
• Lives or lived in the Greater Grand Bend area
• Has been active in the community for most of the past 10 years
• Has exhibited the highest standards of ethics, commitment, ability and innovation
• Has worked collaboratively with others to enrich our community
• Has been a role model and mentor
• Is not currently holding political office

• Write a nomination statement of no more than 500 words outlining the nominee's achievements acid the
reasons for the nomination
• Obtain two signed letters of support (from individuals other than the nominator)
• include a cheque for $100, a donation to the Grand Bend Community Foundation (tax receipt available) in
the name of your nominee
• Submit the statement, letters of support and cheque to the Grand Bend Community Foundation, Box 1150,
Grand Bend, Ontario, no later than September 1, 2011.

What happens next?
• The Community Leader of the Decade Awards Committee will review all nominations to ensure the nominees are eligible for recognition.
• All eligible nominees will be recognized at a gala event on October 28, 2011.
• Each nominee will receive a special award celebrating her or his contributions.
• Guests and others in the community will have an opportunity to make a donation to the Grand Bend Community Foundation in the name of any honouree.
• Each honouree will be profiled in the Lakeshore Advance.
