Huron Family Health Team
Seaforth
To residents without a family physician:
If you or a family member does not now have a family physician, the Huron
Family Health Team in Seaforth is expected to begin shortly.
A Family Health Team is an approach to primary health care that brings
together different health care providers to co-ordinate the highest possible
quality care to patients. The goal of the Huron Family Health Team in Seaforth
is to provide all Huron County residents ready access to a health care
professional.
As it begins, the team is gathering information across Huron County and area
of anyone not now having a family physician.
Registration does not guarantee immediate service. This stage will first gather
the information to determine the medical skills necessary to implement the
Team. As residents register, visits to a health care professional will be
arranged. The Team will make appointments first for those already on
physicians' and clinic waiting lists. Anyone without a family physician is
encouraged to register.
To voluntarily register, be prepared to provide for each resident - name, 911
address, mailing address, nearest town, postal code, phone number, birthdate.
The information gathered will be used solely by the Family Health Team.
To register, phone: 519.887.8886, 519.527.2431, or 519.440.6020.
Forms are also available at the Seaforth and Brussels Libraries or printable
from the frontpage of website www.huroneast.com.
