RHEUMATIC FEVER
There has been a steady de"
cline in the over-all mortality
rate from rheumatic fever and
rheumatic heart disease, al-
though poverty and poor hous-
ing tend to keep the incidence
higher than it might otherwise
be.
The severity of the disease
has been less pronounced, like-
ly because of antibiotic treat-
ment. Most important still,
the incidence of recurrence of
rheumatic fever has been not-
ably reduced by antibiotic
therapy.
The knowledge that rheu-
matic fever is related to a cer-
tain type of infection com-
monly known as streptococcus
(strep) infection, and the use of
penicillin, have provided the
doctor with an effective wea-
pon against the disease and its
recurrence.
The use of penicillin in the
treatment reduces the inci-
dence of rheumatic fever by
eradicating the strep infection.
Penicillin is started as soon as
a case of rheumatic fever is
discovered and is continued
following the acute attack,
either by monthly injection or
daily by mouth, for about X&
years. This helps prevent re-
currence of the streptococcal
infection and rheumatic fever.
Fifty per cent of patients
with rheumatic fever develop
heart murmurs, 'usually early in
the disease, and this indicates
heart damage. Patients with
severe heart involvement are
treated with steroids, or some-
times aspirin is used.
Recurrence of rheumatic fev-
er increases the probability of
heart damage, thus the pro-
phylactic use of penicillin after
an attack lessens the severity
of heart damage by preventing
a recurrence of the disease.
