of Fees for surgery and administration of anaesthesia
• Prescription drugs and medicines ordered or given by a
doctor
• Laboratory Services ordered by a doctor
MAJOR 'MEDICAL BENEFITS
Included in Above Rate
• Appliances, Therapy or related services ordered by a
doctor
• Such Inoculations, Injections and Vaccinations not al-
ready covered •
• Nursing Care, when medically necessary outside the hos-
pital (Registered Nurses ordered by a doctor).

YEAR OR ANY ONE ILLNESS IS GUARANTEED TO A LIMIT
OF $5,000.00 SUBJECT TO THE FOLLOWING:
The Co-operative pays 80% on all Major Medical claims in
excess of $100 each contract year.
