Caring Together for our Communities The South West Community Care Access Centre invites applications for the following positions: CASUAL CASE MANAGERS Successful applicants must demonstrate the following qualifications: • A regulated health professional, preparation at baccalaureate level preferred • Current, active registration or license to practice in Ontario • Minimum of three years relevant recent experience, preferably in community health • Have a sound knowledge of the long-term care system and community resources • Have the knowledge and experience necessary to assess a person's requirements, determine eligibility and develop, evaluate and revise plan of service • Demonstrated communication and interpersonal skills • Basic computer and keyboarding skills • Valid driver's license and own transportation Applications will be accepted until 11:59pm on Sunday, January 27, 2008.
