Workers (HSW/PSW).
• develop goal -directed, outcome -based care plans for personal support and
homemaking with clients and HSW/PSW for all referral sources.
• liaise with all referral sources.
• work as part of the CCAC service team and participate as
a member of agency teams as required.
REQUIREMENTS:
• Registered Nurse
• Computer literate.
• Ability to travel throughout Huron County; occassionally
out of county travel.
• Must be available for "on call rotation" approximately every six weeks.
