Local hospitals may get
more hands-on help.
Kathy Scanlon, executive
director of Community Care
Access Centre (CCAC), told
the Huron Perth Healthcare
Alliance that working togeth-
er is "essential" in a presenta-
tion to the board last night.
"We're in such interesting
changing times. The expecta-
tions are high and the
resources are unsure," she
said.
CCAC is responsible for
home care, patient assess-
ment and connecting patients
to long -term -care facilities if
necessary and providing sup-
port services to caregivers
among other health-related
duties.
About 40 per cent of the
referrals CCAC gets come
from regional hospitals, nine
per cent come from out-of-
town hospitals and 38 per
cent come from community
referrals.
"Our work is based on
partnerships and connecting
people," Ms. Scanlon said.
She would like to see more
collaboration between local
hospitals and CCAC, particu-
larly when a patient is about
to be discharged.
Meantime, CCAC is work-
ing on new initiatives to take
some pressure off the hospi-
tals. She suggested a case
manager could be posted in
the emergency department
and that some patients could
be referred to CCAC and
avoid hospital admittance.
CCAC is also working on a
"step-down site" which
would be a place patients can
recover and undergo obser-
vation, she told the board.
CCAC will reach out to
physicians and ask how it can
do better and asked the
alliance board to think about
what could be done different-
ly, she said.
"When you have a prob-
lem, invite CCAC in to see if
we have a different take on
it," she said.
"I think I sense a real will-
ingness to work together.
Sometimes the hard part is
finding the right way," she
added after the meeting.
