Ministry of Notice to all Family Benefits receipients
Community and Ministry of Community and Social Services
Social Services
Due to the disruption of mail service, the following arrangements have been made regarding
your Family Benefits allowance.
If you normally receive your Family Benefits by a cheque in the mail, your cheque will be avail-
able for pick-up at the Ministry of Community & Social Services, 75 Wellington Street, Clinton,
Ontario, NOM 1L0 on Thursday, November 27 and Friday, November 28, 1997 between the
hours of 8:30 a.m. and 4:45 p.m.
You must show two pieces of identification in order to pick up your Family Benefits cheque.
If you receive your Family Benefits through Direct Deposit, your allowance will be deposited as
usual. Your monthly statement will NOT be available during the mail disruption, but will be for-
wrded to you when mail service returns.
The Drug Benefit Card and Dental Card which you received with your October cheque will be
honoured. No new drug or dental cards will be issued. Please continue to use your current
cards during the disruption in mail service. A new Drug Benefit Card and Dental Card will be
provided with your monthly allowance at the end of the month in which mail service resumes.
If you have earnings or training income, you must report that income BY THE 7TH OF THE
MONTH TO ENSURE CONTINUED BENEFITS.
If you normally receive Family Benefits by cheque, a printed Employment/Training Income
Report form will be given to you with your cheque in the usual way.
If you are on Direct Deposit, you will not be receiving a printed Employment/Training Income
Report form in the mail. A supply of these forms will be available for pick up at your local
Family Benefits office.
The completed Employment/Training Income Report form can be dropped off at your Family
Benefits office in the drop box.
