While recognizing that the level of care consid- ered adequate may vary from a remote, isolated arca of Northern Ontario to the Greater Toronto Arca, it can- not be disputed that "ade- quate" care in Southwestern Ontario in 1996 dictates that emergency care requires the availability of surgery and anesthesia and that obstetrical care requires the availability of surgery and anesthesia. These realities fit well with a simple model for a restruc- tured, integrated hospital sys- tem proposed by Art Clark of Maitland Engineering Services Ltd. This model proposed a 2 level system stnrctured as follows: • Stratford Hospital as an urban, secondary hospital with its own governing hoard and its own budget •a united rural hospital, formed by combining all seven remaining rural sites. with a single governing hoard and its own budget. This united rural hospital hoard could work very close- ly with both Community Care Access Centres in Huron and Perth to integrate our.hospital systems with our home care and long-term care systems so that our structures are truly patient or customer - focused systems. This model also recognizes the historical. truths that hos- pital locations are rationally and reasonably located based on geography, demographics, local needs, weather and access requirements and local resources. Undoubtedly, many chal- lenges will he faced by all rural hospitals in lisht of reduced government funding. Our community hospitals have met all past challenges with success, however: and there is no reason to doubt they will not meet with suc- cess in the future. Under this model, costs are known and can he tixcd. Under the.DHC proposed options, no costs are known and many costs such as the increased costs of transporting patients longer distances by ambulances, the increased cost of killings by physicians (since both the local physician and the hospi- tal site physician will then render the same service with a duplicated killing) cannot he estimated. This sensible yet simple model recognizes the unique, urban realities and strengths of Stratford while, at the same time, it rcmovcs harri- ers so that our country prag- matism, rural resourceful- ness, true community support (financial and othcrwisc)'and community spirit of caring can secure our future health care with local innovation and logical solutions.
