We have full time, temporary and casual Case Manager positions available in both Huron and Perth CCACs. Applicants must: • Be a registered nurse with a current Certificate of Competence from the College of Nurses of Ontario Or • Be a registered health professional and have a degree in a related health science, social science or gerontology And The professional is required to be registered in good standing with the relevant regulatory body. • Have a minimum of 3 years relevant recent experience, preferably in a community setting. • Have a sound knowledge of the long-term care system and community resources. • Have the knowledge and experience necessary to assess a person’s requirements, determine eligibility and develop, evaluate and revise a plan of service. • Demonstrated communication and interpersonal skills • Have a driver’s licence and own transportation. Preference will be given to those applicants with previous case management experience or who have completed programs in case management.
