Cosmetic medicine clinic comes to Wingham

Wingham area residents will
soon be able to have their varicose
veins and face veins treated at the
Wingham and District Hospital
(WDH), instead of going to a larger
centre.
Dr. Mario Cauchi will be
opening a vein clinic at the WDH
on April 18. Dr. Cauchi operates
the Loramar Centre for Cosmetic
Medicine in Goderich. He will be
assisted by his wife Lorraine, RN,
nurse practitioner.
While providing numerous
services at his Goderich clinic,
some of those services will be
offered at the Wingham clinic. The
services provided in Goderich
include slerotherapy which is the
treatment of varicose veins by
injection and not surgery; glycolic
acid facial peals; collagen injec-
tions of facial wrinkles, lines and
scars; and laser treatment of skin
lesions and face veins.
The clinic at WDH will be open
every second Thursday, between 1
and 5 p.m. Appointments can be
made by calling 1-800-896-4430.
Dr. Cauchi noted that a person
does not have to be referred to the
clinic by a doctor and the initial
consultation is free.
Just what are varicose veins and
what is involved in treating them?
Varicose veins and spider veins
are permanently dilated veins.
These veins are not working
properly or adequately to return
blood up the leg to the heart. The
major cause of varicose veins is
heredity.
A person can have inherited
veins with weak valves inside of
them and the person is unable to
change the fact. Prolonged
standing, pregnancy, and weight
gain are also factors which
contribute to the varicose vein
problem.
It is important to note that
varicose and spider veins, once
they start are progressive, which
means they do not get better by
themselves or go away, they get
worse.
There are four methods of
treating varicose and spider veins.
These are through the use of
compression stockings/hosiery,
sclerotherapy, laser treatment, or
surgery.
Sclerotherapy is the process by
which varicose veins are injected
with a solution which destroys the
vein after four to 12 weeks and
blood circulation is restored
through normal veins.
"The solution which we use at
the Loramar Centre for Cosmetic
Medicine are all 'natural'
substances of sugar, salt and iodine
solutions," said Dr. Cauchi. ''We
rarely use chemical injection
solutions."
"The solution we inject reacts
with the inside lining of the vein,
causing it to become inflamed and
swell up inside."
Dr. Cauchi said the effect of this
inflammation is that blood can no
longer flow through the vein, which
leaves the vein inactive. After
several weeks like this, the body
just dissolves (re-absorbs) the vein
and it disappears. He said people
often ask where all the blood goes
if all these varicose veins are
removed with sclerotherapy.
Dr. Cauchi said that first of all,
the varicose vein is not healthy, so
getting rid of it is not making a
person's circulation worse.
Secondly, there are two sets of
veins in the legs and the deep
system of veins is the most
important system because it carries
90 per cent of the blood of the leg.
He added that the deep system
cannot be seen or felt, because it is
below the muscles, next to the
bone.
"The superficial system of veins
is right under the skin," said Dr.
Cauchi. "It is these veins that we
are concerned with because they
become varicose veins."
He said that these veins carry 10
per cent of the leg's blood. These
two systems connect to each other
at designated places throughout the
leg. These connections are called
perforating veins.
The initial assessment of a
person's veins is one of the most
important parts of treatment at the
Loramar Centre. Two people are
involved in the examination, one to
examine the legs, and another to
make a map of a person's veins.
The initial sessions takes about
1 1/2 hours.
"The important thing at the first
visit is to find out which veins are
diseased, what veins are feeding the
problem areas, and which veins are
incompetent," said Dr. Cauchi. "At
the inikial assessment a diagnosis is
made and a treatment plan is
formulated."
He added that photographs and a
photoplethysmography test in leg
circulation along with doppler
studies (ultrasound) will be done at
the first visit.
"At the end of most initial
assessments, we can tell you what
needs to be done," said Dr. Cauchi.
"At the Loramar Centre for
Cosmetic Medicine, we feel it is
important that the patient is totally
informed about their vein
problem."
Dr. Cauchi said the treatment of
varicose veins by laser is the latest
in vein treatment, now offered by
the Loramar Centre. The krypton
laser treats red-coloured
telangiectasia which are difficult to
treat with injections. The laser also
treats face and nose veins.
He said laser treatment is quick,
relatively painless and safe. Dr.
Cauchi added there is no scarring,
bruising or skin discolouration.
Because varicose veins are
inherited, Dr. Cauchi said veins
will continue to make veins, but the
treatment of veins by sclerotherapy,
compression stockings, weight
control, exercise, and vitamins will
keep the veins in check.
Once treated, a person's legs
should be checked every six to 12
months to sec if there are any new
veins.
"At the Loramar Centre, we
believe veins are challenging and
everyone's legs are unique," said
Dr. Cauchi. "We love legs and look
forward to helping you have the
healthiest legs possible."
