After referral, each client is assessed by a Home Care co-ordinator, who will determine that individual client’s eligibility, and what services are required. Once a client is accepted into the program, a co-ordinator will continue to visit on a regular basis, to re-assess and update care plans for that particular client. If a person is not eligible for the Home Care Program for any reason, the Home Care co-ordinator will provide assistance in arranging alternate care. Inordertoprovidethecare needed by its clients, the Huron County Home Care Program contracts professional care from various agencies within the county. Nursing services are purchased from three agencies, The Victorian Order of Nurses (VON), Community Nursing Services at Belgrave, and from the Public Health Nurses of the Huron County Health Unit, while homemaker services are purchased exclusively at this time from Town and Country Homemakers of Wingham. As well, Home Care has on staff several physiotherapists and occupational therapists, and one speech therapist, who provide services in the client’s home, as well as in nursing homes and homes for the aged throughout the county. The Home Care Program can also provide enterostomal therapy for clients requiring it. Home Care also administers the new Integrated Homemaker Program, funded by the Ontario Ministry of Community and Social Services (COMSOC), which provides assistance to frail elderly and physically disabled adults in the county. This program does not require a referral from a physician: anyone who needs assistance may apply. Services provided include the planning and preparation of meals, light housekeeping, laundry and daily personal care for those who require assistance. Information and eligibility may be obtained by calling the Home Care Program (number included at end of article). A third program administered by Home Care is the School Health Care Services program, which began in 1984 and is funded by the Ministry of Health. A physician’s referral is required for each client. Thefinalprogramadministeredby Home Care is the Assistive Device Program, which provides 75 per cent of the cost of selected and prescribed assistive devices required by clients 24 years of age or younger throughout the county. The Huron County Home Care Program is fully covered by OHIP premiums, and no further payment is asked of any client who meets the eligibility requirements for the program.
