Huron Hospitals Discuss
Need for Additional Beds
Representatives of Huron hospi-
tals appeared before Huron Coun-
cil Thursday morning .to discuss
with council the problems facing
the hospitals in providing funds for
necessary expansion. Spokesmen
for the group were A. M. Knight,
Clinton, and A. Y. McLean, Sea -
forth.
The five hospitals at Seaforth,
Clinton, Goderich, Exeter and Clin-
ton were represented by A. M.
Knight, H. C. Lawson, of Clinton;
A. Y. McLean, Scott Habkirk and
Earl Dick, Seaforth; J. H. Kinkead
and Leo Waizak, Goderich, and
Roy Cousins, Wingham.
The brief set out the picture as
seen by the hospitals and express-
ed appreciation for the interest
shown by members of county coun-
cil in the problems facing the
boards.
Generally the hospitals have
been established by interested per-
sons who formed Boards of Trus-
tees. The hospitals have been con-
ducted on a non-profit basis as a
service to the communities. The
Boards are proud of the achieve-
ments of the past and are thank-
ful for the support which has been
received during the bast years.
The financial support given through
donations has been deeply appreci-
ated.
The introduction of the National
Health Plan has changed the op-
eration of the hospitals. While this
has assisted, there are many prob-
lems facing hospital boards. In
order to understand the complete
story, it is important to distinguish
between a hospital's "operating"

costs and a hospital's "capital"
costs and where the hospital ob-
tains the funds to offset these
costs.
When a patient is admitted to
hospital, he is charged so much a
day for the care he receives. (If
insured under Ontario Hospital In-
surance, this charge per day is re-
imbursed to hospitals by the On-
tario Hospital Services Commis-
sion.) It is this charge per day
which covers the hospital's oper-
ating costs and it includes such ex-.
penses as nursing care drugs,
medical and surgical supplies, ad-
ministration, salaries and wages
of the hospital's staff, maintenance,
etc. The daily rate is determined
at the beginning of the year and
is applied to all patients. At the
close of the year, a final account-
ing is made to insure that the
charge has been correct. If the
rate was too high, the hospital
must remit to the Commission any
overcharge. You can understand
from this that the rates will vary
in different hospitals, and that
there will be no funds accumulat-
ing in hospitals' operating ac-
counts.
Lack Capital Funds
The capital costs of a hospital
include that amount of money nec-
essary to finance new construction,
the extension of existing facilities
and the cost of new equipment.
Funds for this purpose in the past
have been furnished by municipal
grants, by fund raising campaigns
and by bequests. Since the incep-
tion of the National Hospital Plan,
federal and provincial grants have
been made. These grants now total
$4,000 per bed and a small grant
for auxiliary space, payable on
new construction. As a Matter of
interest, it is estimated that it av-
erages about $14,000 per bed to in-
crease existing hospital facilities,
taking into consideration the auxil-
iary areas which must be added
when any expansion is made. Thus,
it is fair to. state that the provi-
sion of the greater portion of the
hospital's capital costs is the re-
sponsibility of the local commun-
ity. These funds can only be rais-
ed by donations, bequests and lo-
cal taxation.
Set Out Plans
Hospital Boards are desirous of
providing adequate hospitalization
for the community. Since it is dif-
ficult to raise the funds for con-
struction, there is no attempt to
build more than is required. Un-
der present regulations, the Board
must secure the approval of the
Ontario Hospital Services Com-
mission to extend hospital facili-
ties. The following brief survey
details the problems in this coun-
ty. -
Clinton
Clinton Hospital has accommo-
dation for 40 patients. The On-
tario Hospital Services Commis-
sion has approved an extension of
20 beds with provision for extend-
ed services. Preliminary plans
have been prepared for this exten-
sion. The cost is estimated at $222,-
213. It is estimated that the fed-
eral and provincial grants will
amount to $130,860. The remaining
portion, $91,353 must be secured
before this project can be under-
taken.
Exeter
Exeter has accommodation for
38 patients. Approval has been
given for the addition of 16 beds.
The cost of the project is estimat-
ed at $170,000 with estimated grants
of $65,000.
Goderich
The Alexandra Marine and Gen-
eral Hospital in Goderich has ac-
commodation for 95 patients; this
includes a chronic convalescent
wing of 24 beds• Four years ago,
there was completed an addition
of 41 beds and many service ar-
eas; operating rooms, obstetrical
wards, nursery, laboratory, kit-
chen, admitting area, X-ray rooms
and an emergency ward. The cost
of this addition including furnish-
ings was $563,000.
The plan of the Board which' has
been discussed with the Ontario
Hospital Services Commission is
the removal of the original build-
ing and the construction of a 40 -
bed addition to replace it. An al-
terate plan was to make major
changes in the old building. These
alterations are deemed to be too
costly. The cost of an addition at
$8,000 a bed would be $320,000.
The federal and provincial grant
would amount to $160,000. (Note
the estimated cost of $8,000 would
probably provide these facilities
since the major expenditure for
auxiliary area was made at the
time of the last addition.)
Seaforth
Scott Memorial Hospital at Sea -
forth is experiencing demands for
accommodation. This has led to a
complete review of the entire hos-
pital
ospital plant which has been under
way by the Board for nearly two
years.
The hospital is accommodated
within a former residence built 80
years ago and converted to hospi-
tal purposes in 1927. To this was
added a frame and brick addition
in 1949. At the moment, it has an
established capacity of 37 beds.
Having regard to problems of
financing capital expenditures, the
Board's first thoughts were to car-
ry out alterations to the existing
building and to construct an addi-
tional wing, increasing the capac-
ity to 50 beds, Approval for such a
proposal could not be obtained
from the Ontario Hospital Servic-
es Commission and as an alterna-
tive„ the Commission advised that
consideration should be given to
construction of a new hospital
building. On the basis of this de-
cision by the Commission, the
Board of Scott Memorial Hospital
commenced plans for a new build-
ing on a new site. The existing site
did not lend itself to new construc-
tion.
Since that time, a site on No. 8
Highway, adjoining Seaforth on
the east, has been purchased. The
Ontario Hospital Services Com-
mission has given approval for the
construction of a hospital of 40
beds with provision for, necessary
services. The question of increas-
ing the capacity to 50 beds will be
reviewed on completion of a sur-
vey to determine the requirements
which is now in progress. The firm
of Agnew, Peckham and Associ-
ates, hospital planning consultants,
have been retained and is carry-
ing out a complete survey of the
area to insure that adequate ser-
vice will be provided for in the
new hospital structure which is
contemplated. The consultants are
working with the architects which
the Board have appointed, and it
is anticipated that a draft of the
survey will be available by the
end of the year and that prelim-
inary plans will be available in
May,- 1961.
Estimates of cost arrived at
through discussions with consult-
ants, the architects and officials
of the Ontario Hospital Services
Commission suggest an amount of
up to $14,000 per bed or perhaps
$700,000 in all. Of this, approxi-
mately $4,000 per bed plus cer-
tain grants for service areas and
other facilities can be anticipated
from the provincial and federal
governments. The balance of at
least $400,000 must be secured be-
fore the project can be commenc-
ed.
Wingham
Wingham General Hospital has
accommodation for 95 patients.
During the past three months the
average number of patients per
day has been 126. This hospital
was extended a few years ago at
considerable cost. The new addi-
tion provided much needed patient
accommodation and facilities, kit-
chen, laboratory, etc., for a much
larger hospital. The Board realiz-
es the need for a new addition and
has discussed the problem at var-
ious times with the Ontario Hospi-
tal Services Commission, and has
held discussions with architects.'
